Brief Grief Questionnaire

The questions in this scale ask you about over the last two weeks, how often you have been bothered by the following problems. In each case, you will be asked to indicate by selecting the answer that correlates to how often you felt or thought a certain way.
0 = Not At All, 1 = Somewhat, 2 = A Lot
Q1: How much are you having trouble accepting the death of ______________?
Q2: How much does your grief still interfere with your life?
Q3: How much are you having images or thoughts of _____________ when s/he died or other thoughts about the death that really bother you?
Q4: Are there things you used to do when ______ was alive that you don’t feel comfortable doing anymore, or that you avoid? Like going somewhere you went with him/her, or doing things you used to enjoy together? Or avoiding looking at pictures or talking about _________? How much are you avoiding these things?
Q5: How much are you feeling cut off or distant from other people since _________ died, even people you used to be close to like family or friends?

A score of 4 or more suggests an individual may have complicated grief

Privacy & Confidentiality

All enquiries and counseling sessions are private and confidential. During supervision, identities of the clients are not revealed. Only in exceptional circumstances would confidentiality be broken. The exceptional circumstances would be if :

  • There is a clear and immediate danger to an individual or to society
  • There is a legal compulsion to make the disclosure and a failure or refusal to comply will constitute a criminal offense or contempt of court.

Your counselor will explain to you in detail on your first counseling session.
